I would like to thank you for this comment. Although I do not know the figures by heart, I can say that the sensitivity of bronchoscopy with TBB was around 68%. Although all participants were routine practicioners, and although they were all instructed to do TBNA, TBB and EBB, it was very surprising to see that in many cases, no TBB were taken (for very various reasons). These data nuance a lot about the value of guidelines. About BAL : BAL was left at the discrecy of the participating phycician. Because this was an implentation trial, and because of methodological reasons (difficult standardisation between 15 hospitals) we choose not to do BAL routinely or to measure the performance of BAL. So we have no conclusive data on that point available.

eus vs ebus
I do not have a clear answer to your question, however, while EBUS was performed by 13/15 participating chest physicians (with a variable degree of experience); EUS was only performed by a few (I do not remember by heart, but is is not more than 3-4; all with a high level of experiene). So 'experience'bias might have played a role, rather than an intrinsic reason favouring EUS. kurt

THE ERS ATS Wasao statement advices to take 4 TBLB The overall yiled of bronchoscopy in assesing granulomas wasaround 60 % when biopsies were taken.What was the number of biopsies taken and from which location ( EBB/ vs TBNA vs TBLB) Did BAL play any role?
