COPD and activities of the daily life: impact of the disease severity results of an epidemiological study

Abstract

INTRODUCTION: Patients with COPD develops severe exercise limitation which can interfere with daily activities.
AIM: To evaluate the impact of COPD on the patient‘s functional capacity using a structured questionnaire: the London Chest Activity of Daily Living Scale(LCADL
METHODS: Multicentre Observational cross-sectional study. Inclusion criteria: Patients with a confirmed diagnosis of COPD who were consecutively recruited by a pulmonologist in the out setting clinic. In all patients dyspnoea was evaluated using the modified MRS scale and all of them fulfilled the LCADL in order to evaluate their degree of limitation for the activities of daily life.
RESULTS: We studied 3608 patients who were classified in 2 groups: group A 1596 patients mild or moderate disease and group B: 2012 patients with severe or very severe disease. Patients in group A shown a significantly higher proportion of dyspnoea I or II, while III IV and V dyspnoea degrees were more common in patients in group B ( I: 10% vs. 2,6%; II 49% vs. 24,5%; III 31% vs. 36,3%; IV 9,3% vs. 27,1%; V 0,8% vs. 9,6% ; p<0,001). A significant difference between group was also found in the global LCADL scale as well as in their different domains (Global: group A 21.4 group B 29, 6; self-care: 6,6 vs. 9,08; domestic activities 5,86 vs. 8,77en EPOC; physical condition 4,64 vs. 5,86; leisure 4,31 vs. 5,88; p<0.001 in all cases)
CONCLUSSION: Our results confirm that, in patients with COPD there is a strong association between the severity of the disease and the physical limitation. This limitation has an important impact in the activities of the daily life.
