Subjective sleep quality and drowsiness of nurses working in shifts in a large general hospital. A pilot study

Abstract

Sleep management on shift workers has become a great health occupation problem. Nursing is one of the professional categories constantly exposed to large periods without sleep and high levels of stress. Recent studies have shown the relationship between medical and nursing procedures mistakes and sleep deprivation. Therefore, the aim of this study was to verify the subjective sleep quality of nursing team working in night shifts.
We have carried out a primary evaluation in thirty-seven night nurses and nurse assistant during their shifts. The Pittsburgh Sleep Quality Index (PSQI), Epworth Sleppiness Scale (ESS), anthropometric data and Mallampati Score were assessed. Descriptive statists, Mann-Whitney and t-test were used for analysis (p<.05). It was considered for first analysis just one group of nurses and afterwards two groups separated by complexity of work unit – clinics unit and emergency unit.
Average of PSQI and ESS were 7.27 ± 4.15 and 8.81 ± 4.63. Frequency of "poor sleepers" was 62.16% and "daytime sleepiness" was 37.14%. When the data has been analyzed for each group we have not found significant differences between frequencies of poor sleep or drowsiness as well as absolute values for PSQI and ESS between groups (p>.05).
Shift work in nursing team seems to be linked to poor sleep quality and there were not relation with unit work complexity. We can conclude that there is a major necessity to improve sleep quality in these subjects helping to prevent occupational accidents and to avoid procedures mistakes.
