Morbidity and mortality in patients with chronic obstructive pulmonary disease and cachexia

Abstract Aim: To determine the one-year mortality and morbidity in patients with chronic obstructive pulmonary disease (COPD) and cachexia . Material and Methods: Study group was composed of 45 COPD patients with BMI < 21 kg/m² . Lung functions (spirometry, arterial blood gases) and body composition assessments with bioelectrical impedance were performed for all patients. We also measured biceps, triceps, and abdominal skin fold thicknesses by a caliper. Patients were re-evaluated about frequency of emergency admissions, hospitalizations, acute exacerbations, and mortality was assessed at the end of the first year.Results: Mean age of the study group was 65.6±9.6years, The laboratory findings were as follows: mean PaO2 : 69.1±7.7 mmHg, PaCO2 : 43.1±4.0 mmHg, SaO2 : 93.3±2.1 %, FEV1 : 43.0±13.6 %, FEV1/FVC: 45.0±8.0 %, DLCO: 50.8±23.5 mmol/kPa/min. Mean BMI was 19.2±1.5 (14.8-21) kg/m2 , triceps, biceps and abdominal skin fold thicknesses were as follows: (5.0±1.7; 4.2±1.3 and 7.6±2.6mm). Mean fat mass percentage was 20.2±6.7 %. One year mortality was 8.9 % (n=4). 61 % had acute exacerbation with a frequency of 1.4±1.4; 51.2 % had emergency admission with a frequency of 1.0±1.2; 41.5 % were hospitalized with a frequency of 1.6±0.5. When we compared the patients according to abdominal skin fold thickness, patients with abdominal skin fold thickness <10mm (Group 1, n: 33; % 73.3) and ≥10 mm (Group 2, n: 12; % 26, 7) Group 2 had higher one-year mortality rate (p=0,022). Mean abdominal skin fold thickness was 9.75mm in patients who died in one year period. Conclusion: Mortality ant morbidity in COPD patients with cachexia were high. Increased mortality rate was associated with abdominal skin fold thickness.
