The effect of comprehensive pulmonary rehebilitation for anxiety and depression in patients with COPD: is there any difference between early and late stages of the disease?

Abstract

Introduction:Psychological and emotional problems are common in patients with COPD and secondary impairments such as depression and anxiety usually influence their pulmonary symptoms.
Aim:To assess the emergence of anxiety and depression in patients with early(GOLD;I+II) and late(GOLD;III+IV) stages of COPD and the effect of comprehensive pulmonary rehabilitation (PR) for anxiety and depression.
Method:We evaluated 55 patients with COPD who participated in our outpatient PR program.SGRQ and HADS are used for the assesssment of anxiety, depression and quality of life.
Results:The early stage group‘s (n=28) mean FEV1% predicted was 42.46 ± 17.4 and the late stage group‘s (n=27) was 27.33 ± 8.2. SGRQ total, activity and symptom scores were more impaired in the late stage group, though there were no stastistically significant difference between the two groups. Psychometric assessments with HADS revealed similar impairment levels in both groups and there were no stastistically significant difference between the two groups. After PR, SGRQ total scores, activity and impact scores revealed an improvement in both of the groups (p<0.001), whereas the difference of SGRQ symptom domain in the early stage group was not found stastistically significant. Also HADS scores decreased significantly in both of the groups (p<0.001).
Conclusion:The degree of airway obstruction does not explain anxiety and depression.All COPD patients should be evaluated for psychological impairment. Comprehensive PR,significantly reduce anxiety and depression in patients with COPD.
