Abstract

Aim: To explore the attitudes of people with COPD to exercise, and reasons for non-concordance with exercise maintenance post-pulmonary rehabilitation.
Methods: Qualitative study using a focus group of six COPD patients recruited through the patient support group Breathe Easy. The data was analysed using inductive thematic analysis and triangulated by peer and member checking.
Results: From the focus group discussion, three main themes were identified:
Effects of exercise
Facilitators to exercise
Barriers to exercise
The effects of exercise were perceived to be mainly positive by the group, although tiredness was also mentioned.
Subthemes of facilitators to exercise were: encouragement, company, professional support (to grade exercises and create a routine), goal-setting, personal attributes (incl. persistence and past exercise history), and availability of a range of exercise options.
Subthemes of barriers to exercise were: changing health status (due to exacerbations and co-morbidities), fear, lack of support (at home and in hospital), and environment.
Conclusions: The views of this focus group suggest that service providers should seek to assist people with COPD in strengthening exercise facilitators and overcoming barriers to exercise. As individuals‘ needs change according to their current health status and circumstances, support structures need to be in place and communication between patients and healthcare professionals needs to be ongoing.
A range of exercise options needs to be available and combined with professional support, goal-setting, encouragement, company and a suitable environment.
More research is needed into concordance with and cost-effectiveness of specific exercise maintenance regimes.
