Comorbidities and disease specific health status in young and old COPD patients

Abstract

COPD is an important cause of morbidity and disability in elderly and is often accompanied with other chronic diseases. These comorbid diseases might influence scores on COPD health status questionnaires. The aim of this study was to evaluate the relationship between comorbidities and disease specific health status in patients with COPD. Methods: Consecutive COPD patients visiting an out-patient clinic were evaluated by spirometry, BODE, and health status using the CCQ and the SGRQ. Comorbidities were assessed by the Charlson Comorbidity Index (CCI) and the Chronic Disease Score (CDS). Results: 158 patients were divided into 2 age groups (above or below 65 years)(table 1). In young patients, there were no significant correlations between health status and comorbidities. In the elder group, health status and comorbidity correlated weak (CCQ total score and CCI r= 0.21, p<0.01 and SGRQ total score and CCI r= 0.27, p<0.01). In both age groups, there was a strong correlation between CCQ and BODE (r~0.6, p<0.01) and SGRQ and BODE (r~0.4 p<0.01).

Conclusion: Comorbidity scores only weakly correlate with heath status scores on the CCQ and the SGRQ. This suggests that both the CCQ and the SGRQ are able to measure specific COPD related health status.
