Difficulties in diagnosis of COPD in patients with advanced peripheral artery disease

Abstract

Smoking cigarettes is the main risk factor of chronic obstructive pulmonary disease (COPD) and peripheral artery disease (PAD). In the group of patients with PAD physical activity is reduced due to intermittent claudication. It may cause delayed COPD diagnosis in this group of patients.
Aim of our study was to evaluate difficulties during COPD diagnosis among patients suffering from severe PAD hospitalized in surgery department due to limb ischemia.
We examined 64 patients, mean age 62 years hospitalized in surgery department due to symptoms and complications of advanced PAD (at least IIb stage according to Fontaine scale). They responded to the questionnaire about symptoms of COPD and dyspnea using MRC scale was assessed. All 64 patients had done physical examination and spirometry. There were either current-smokers or ex-smokers in examined group (the average number of pack years was 50,45).
Spirometry-defined COPD was present in 39% of subjects. During the study we diagnosed 16 patients with COPD; 9 patients had been formerly diagnosed with COPD. All patients with COPD declared at least one symptom of disease. It was not possible to assess dyspnea using MRC scale in 44% of 34 patients who reported dyspnea.
In all group of patients with COPD negative prognostic factors of the disease were common: 60% of patients were current smokers, 52% had cardiac arterial disease, 52% arterial hypertension, 40% BMI< 21kg/m2, 36% heart failure.
COPD is a frequent disease among patient suffering from severe PAD.
In group of patients with advanced PAD MRC scale is not proper instrument to evaluate dyspnea intensity.
Patients with advanced PAD seem to be good candidates to perform spirometry as a screening.
