Routine left adrenal visualization is certainly advised!
Thank you Jouke, Routine left adrenal visualization is certainly advised. In every patient we routinely start the procedure by advancing the scope into the stomach and localize the aorta, tr. coeliacus, left kidney and left adrenal. Tr coeliacus metastases are also reported and can be aspirated, left adrenal abnormalities can easily been seen and with onsite cytological evalation a M1 status can be confirmed instantly. We do advise to proceed with mediastinal lymfenode FNA if applicable and use a new needle to avoid false positive N-metastases. Without the presence of a solid intrapulmonary tumor however, the guidelines voor adrenal incidentaloma should be followed and FNA is not adviced. The right adrenal however is difficult - do you have any recommendation for that? Erik

Congratulations These patients were investigated by EUS based on suspected adrenal glands by imaging. Do you recommend to visualize the left adrenal gland routinely (also when not enlarged or PET +) There is one case reoert (Ringelbeek et al) who found an unexpected m1 in such case
