Socioeconomic status and obstructive sleep apnea hypopnea syndrome (OSAHS)

Abstract

Background: Obstructive Sleep Apnea Hypopnea Syndrome (OSAHS), a common disorder, is characterized by repetitive episodes of cessation of breathing during sleep.
Objectives: The aim of this study was to investigate whether there is an association between socioeconomic status, occupation and OSAHS.
Methods: A study was conducted in the sleep disorders laboratory in "Sotiria" Hospital for the Diseases of the Chest in Athens. A sample of 366 subjects was screened for OSAHS and the presence was defined after polysomnography by an apnea-hypopnea index (AHI) >5. A detailed questionnaire was used for social characteristics (ie marital status, income, educational level, occupational status).
Results: The influence of age , body mass index (BMI) and social characteristics on AHI was examined by multinomial logistic regression. The following factors remained independent risk factors for the presence of OSAHS: (1) Gender: odds ratio (OR) 8.73 (95%Cl 2.98-25.60) for male gender; (2) Obesity: OR 1.17 (95%Cl 1.08-1.27); (3) Occupational status: administrative and secretarial occupation: OR 8.59 (95%Cl 1.94-37.98) p=0.005, skilled trades occupations: OR 5.42 (95%CI 1.3-22.66) p=0.021, machine operatives: OR 14.62 (95%Cl 1.49-143.41) p=0.021 and elementary occupations: OR 7.36 (95%Cl 1.96-27.63) p=0.003.
Conclusion: In this study, factors associated with the presence of OSAHS include gender (men), obesity (BMI) and occupational status. Increased risk was found for machine operatives, elementary occupations and administrative and secretarial occupations.
