Abstract

The old age is not a contraindication to perform respiratory rehabilitation programs also in severe and very severe COPD patients with respiratory insufficiency. The aim of the study was to evaluate the quality of life (QoL) of these elderly patients (C >74 y) in comparison with two younger groups of patients. (A <65 y; B 65-74y). We underwent 109 COPD patients (A 24;B 47;C 38) to MRF-28 questionnaire (ERJ 1999,13,1293) before and after three weeks of respiratory rehabilitation (6/7 days a week). At the end of the program we observed a significant total score reduction in all the groups: A from 44 to 27; B from 44 to 27; C from 44 to 31. However if we consider the three different components of the total score in MRF-28 we observe that in C group only daily activity improves after treatment while cognitive function and sense of general invalidity don‘t show significant changes. In A and B group, the score of daily activity and sense of invalidity show a significant decrease. Little changes have been observed in score of cognitive functions in all the groups but in A group, obviously, the base score was very lower than in B and C group (19 vs 27 vs 36). These data confirm the utility of a respiratory rehabilitation program also in elderly patients but advise to analyze with accuracy all the different components of a QoL questionnaire, overall in aged patients in which the sense of invalidity could assume a different mean than in younger one.
