Improvement of quality of life and cost effectiveness by an outpatient education program for patiens with COPD

Abstract

Background: Up to now the role of structured patient education programs for patients with COPD is critically discussed. In a previous study a reduction of exacerbations was found using a 4x2 hours structured education program for patients with mild to moderate COPD. In the present study we examined Quality of life of the patients and performed a cost-benefit analysis of this program. Methods: 93 patients with COPD (age range, 47-75 yrs.) were randomly subdivided into an education group (n:50) and a control group (n:43). The education group participated in an 4x2 hrs lasting education program. Main items were correct inhalation technique, management of the exacerbations, self-monitoring and adaptation of the medication to the severity of the disease. The equally treated control group got a sham-training. quality of life as assesed by the St. Geroges Respiratory Questionnaire (SGRQ) and costs due to the disease were calcualted 6 month before and after the intervention. Results: The total score of the SGRQ significantly decreased from 45.3 + 9.4 before education to 40.4 + 10.4 (p<0.001) thereafter, whereas no changes for the control group were observed. During the six month period before education, a mean total amount of 3000€ per patient was calcualted for the management of COPD. Durng the 6 month after education these costs could be reduced to 2125.38€ including the additional costs for performing the education, whereas the corresponding costs of the control group increased to 3843.87€. Conclusion: On the basis of these results this outpatint education program for patients with mild to moderate COPD improves Quality of life of the educated patients and is cost effective.
