Comparison of the effectiveness of two schedules of pulmonary rehabilitation in patients with severe COPD

Abstract Background: Pulmonary rehabilitation (PR) is an effective treatment in patients with severe stage of chronic obstructive pulmonary disease (COPD), with the aim of reducing dyspnoea and improving exercise capacity. Many schedules to perform PR are available. We compared the effects on exercise capacity and pulmonary functions of two schedules in patients with similar severity of COPD. Methods: Thirty patients were included in the study, 15 (9 males, 6 females, mean age 71.8 years) undergoing a program in 8 visits (two per week) in a day hospital setting (group A), and 15 (8 males, 7 females, mean age 72.6 years) undergoing a program in 10 daily visits in an in-hospital setting (group B). Both schedules included the same components (physical activity, callisthenic exercises, respiratory exercises, and education on COPD). Before and after PR were measured the meters walked during a 6 minutes walking test (6MWT) and the FEV1 as indicator of pulmonary function. Results: In group A meters walked during 6MWT were 256 ± 94 before PR and 341 ± 91 after PR, and FEV1 values were respectively 36.4% ± 7.5 and 41.1% ± 7.3 of predicted. In group B meters walked during 6MWT were 251 ± 61 before PR and 259 ± 61 after PR, and FEV1 values were respectively 40.7% ± 12.4 and 44.9% ± 16.6 of predicted. The only significant difference was detected in the increase in mean meters walked by group A. Conclusion: The schedule with two visits per week with a duration of one month seems more effective than the schedule with 10 daily visits in improving the exercise capacity in patients with severe COPD.
