Results of surgical management of pulmonary adenocarcinoma in 1a stage (T1N0M0): lessons learned

Abstract

Background. Lung cancer of in the advanced countries is guideing localization in structure of oncological diseases. It is established, that only in time the made surgical operation in an early stage of disease yields the good long-term results.
Aims. To define optimum volume of surgical treatment of pulmonary adenocarcinoma in 1a stage.
Materials and methods. Within 1963 on 2008 yy there 106 patients with pulmonary adenocarcinoma in 1a stage were treated, including 76 male and 30 female. The patients age varied from 25 to 74 years. In 103 cases it is diagnosed peripheral adenocarcinoma. In 3 cases there were central tumors. The diagnosis before operation was verified in 32 cases. All patients were operated. Lobectomy was performed in 86 cases (81,1 %). Precision resection and segmentectomy were made in 20 cases.
Results. Postoperative complications included 9 cases of residual cavity, 3 cases of pneumonia and 1 cases of lung abscess. Lethality rate was 0,9 %. Long-term results within 6 months to 33 years are studied for all patients. 5 years survival rate was 75,6 %.
Conclusions. To our opinion volume of resection does not influence on long-term survival rate. Optimum operation for 1a stage pulmonary adenocarcinoma is lobectomy however for patients with advanced accompanying diseases quite justified it is possible to consider a resection of easy smaller volume.
