Effectiveness of a home-based maintenance program following a conventional outpatient rehabilitation in COPD

Abstract In COPD patients, outpatient pulmonary rehabilitation (PR) improves exercise capacity but benefits are generally lost if exercise is not maintained. Methods : we evaluate in 30 stable COPD (age 64.8 ± 10), the effects of a PR program comprising a 3 months outpatient period (OP) followed by a 6 months home-based maintenance period (HB). OP consisted in 3 sessions/week for six hours a day (50 min of exercise on cycloergometer, 30 min of respiratory therapy, 45 min of body building, 45 min of gymnastic and swimming and 45 min of relaxation). HB consisted in 3 sessions/week of 50 min training on a cycloergometer with a regular control of respiratory therapy and training by physiotherapists. Measurements were made at inclusion (t0 ), at 3 months OP(t3 ) and after 6 months HB (t9 ) and comprised a medical examination, anthropometric parameters, pulmonary function tests (PFT), and evaluation of exercise performance (exercise testing and 6 min. walk test), dyspnea and quality of life (QOL ). Results : Physical condition and QOL improved during the outpatient PR program but also during the home-based PR program with: a) an increase of maximal workload (p <0.05); b) an increase followed by a maintenance of VO2 max (p <0.05); c) an increase followed by a maintenance of ventilatory threshold (p <0.05); d) an increase followed by a maintenance of walking distance (p <0.05); e) improvements in all domains of QOL (p<0.05). Conclusion: in this prospective trial, the effectiveness of a 6 month structured HB to sustain the benefits obtained by an OP were demonstrated. At our knowledge, this is the first trial to demonstrate that outcome measures can still additionally improve during a successive HB.
