Abstract Background: System influence of COPD includes respiratory muscles weakness. Respiratory training with PID directed to respiratory muscle training via expiratory and inspiratory resistance.Aim : To evaluate the effectiveness of PID in COPD patients.Methods : 30 COPD patients, stage III and IY were randomized in 2 groups. Group 1 (64,7+ 7,0yrs) received ICS, LABA and tiotropium bromide. Group2 (64,0+ 11,0yrs) - the same therapy+ PID. 6MWT, respiratory muscles force and endurance, MOS SF-36 and lung function were evaluated at baseline, after 2 weeks and 3 months.Results: In group 2 there was the significant improvement in lung function (ΔFEV1-11+ 6,0% (p<0,01), ΔPEF-10+ 4,3% (p<0,001)), physical tolerance (Δ6MWT)-62+ 13,4 m (p<0,01)), QoL parameters (ΔPF-12,7+ 6,7% (p<0,05), ΔRP-17,5+ 5,5% (p<0,05), ΔCH-27,5+ 12,4%(p<0,01)). In group 1 results were lower: ΔFEV1-3,1+ 0,4% (p<0,05), Δ6MWT-29,8+ 9,6m (p<0,01), ΔPF-6,5+ 1,8% (p<0,05), ΔCH-20+ 8,7% (p<0,001). Conclusion: Breath muscle training using expiratory and inspiratory resistance significantly improved lung function, physical tolerance and QoL parameters in comparison with the medication therapy only.
