The comparison of transbronchial biopsy and endobronchial biopsy in diagnostic of pulmonary sarcoidosis

Abstract

The aim of the study was to determine a value of endobronchial biopsy (EBB) in the diagnosis of pulmonary sarcoidosis, and to know whether the EBB could increase a diagnostic efficacy of fiberoptic bronchocopy (FOB) with transbronchial biopsy (TBB).
Material and methods. There were examined 76 patients with a presumptive diagnosis of pulmonary sarcoidosis. All the patients have X-ray and CT, ECG and spirometric examinations. During the FOB we estimated the bronchial mucosa state and obtained EBB from upper and middle lobe bronchi and the TBB performed under a conventional methodology. The TBB was performed in 54 hospitalized patients and in 20 patients examined in the day hospital. The biopsy results were estimated positive in cases of revealing anecrotic granulomas.
Results. The EBB was positive in 40.1% of the patients, moreover in a third of the patients for whom the TBB was not performed. The EBB results were positive in all the patients presenting mucosal irregularities in FOB. The EBB was positive in 43.7% of patients with thickening and vascularization of bronchial mucosa and in 33.3% of patients presenting no pathological changes of mucosa.
Conclusion. In patients with pulmonary sarcoidosis EBB allows to reveal the pathological changes of bronchial mucosa in more than a third of patients, and in a third of patients underwent no the TBB. Positive results of EBB were obtained in 56.5% of patients with positive TBB.
