Fringe manifestations in sarcoidosis and side effects of corticotherapy

Abstract

Our study was undertaken on a number of 34 cases of sarcoidosis admitted to the Clinic of Pulmonary Diseases Iasi between January 2004 and July 2005.
We took into consideration several aspects, including distribution based on sex, age, territory, extrapulmonary involvement in sarcoidosis, role of clinical and laboratory findings, treatment as well as side effects of corticotherapy.
Our results revealed data similar to the literature, showing a female predominance (22 patients; 64,7%) vs. male patients (12 cases; 35,3%), age average being 43,38 yo (between 21 si 72 yo). Clinical as well as other investigations revealed multiple extrapulmonary involvement in sarcoidosis, including ocular manifestations in 5 cases (14,7%), cutaneous in 4 (11,76%), hepatic and splenic involvement in other 4 cases, cardiac manifestations in 3 (8,81%) as well as osteoarticular, cerebral, renal and parotidian involvementOf the 34 patients, 21 were treated with cortison (61,76%), side-effects being reported in 10 cazuri (29,41%), involving gastric and duodenal ulcer, secondary osteoporosis, Cushing syndrome, miopathy and steroid diabetes, etc.
The results obtained from our study are similar to the data from literature, proving the wide spectrum of organic involvement in sarcoidosis and the requirement for a careful, step by step investigation in order to achieve a correct diagnosis; we also have to consider the balance between benefits and side effects of corticotherapy in order to avoid all possible complications.
