Early diagnosis of extrapulmonary sarcoidosis

Abstract

The prevalence of extrapulmonary sarcoidosis has been estimated in retrospective studies. It has not been determined how early has been reached the diagnosis in the progress of the disease.
Aim of our study was the early diagnosis of extrapulmonary sarcoidosis in a prospective study.
Methods: A careful examination with particular interest to the possible extrapulmonary signs and symptoms of the disease was carried out in 305 consecutive sarcoid patients seen between 2000-2004 at the outpatient sarcoid clinic of our hospital. Many of them had already been examined by other doctors.
Results: Extrapulmonary manifestations of sarcoidosis were presented in 96 patients (31.4%). The most frequent extrapulmonary presentation was due to skin lesions 54 pts (17,7%) and peripheral lymph nodes 18 pts (5,9%). Less frequent were uvlitis 12 pts (4%), pleural effusion 4 pts (1,3%) and others 10 pts. An early diagnosis was reached in 45 from 96 pts with extrapulmonary disease (46,8%). In the other 51 (53,1%) the diagnosis was reached later with an interval raging from 1 till 8 years. 83 pts had also pulmonary sarcoidosis. The median follow up was 29 months.
Conclusion: Extrapulmonary sarcoidosis is common but may go unrecognized for years. Pneumonologists must have in mind the extrapulmonary presentation of sarcoidosis.
