Long-term effects of pulmonary endarterectomy (PEA) on clinical and cardiopulmonary function status

Abstract Chronic thromboembolic pulmonary hypertension (CTEPH) can be successfully treated by PEA but there are few prospective studies on long-term cardiopulmonary function, particularly on exertion. Between 1994 and 2006, 140 patients were treated with PEA at Pavia University Hospital; during the follow-up, 3 patients had a second PEA, 3 had transplant, and 27 died (16 within the first 3 months). In the table clinical and functional data of the 41 patients followed-up for at least 4 years (mean 5.5±1.3) are reported. Nearly all the improvement in hemodynamic, gas exchange and clinical status was achieved in the first 3 months and persisted at the end of follow-up. Vital capacity and distance at exercise testing further improved over time (post hoc analysis). In conclusion, in patients with CTEPH, persistent clinical and functional improvement can be achieved by PEA.
