Hyperglycaemia and its association with outcomes in people admitted to hospital with acute exacerbations of chronic obstructive pulmonary disease

Abstract

The effect of blood glucose on outcomes from acute exacerbations of chronic obstructive pulmonary disease (AECOPD) has not been established. We therefore determined the relationship between blood glucose concentrations, length of stay and therapy selections in people admitted with AECOPD. Methods: Data was retrieved from patients admitted with ²AECOPD without pneumonia² in 2004-5. Patients were divided by blood glucose history. Normal subjects (Group A, n=66, 67.8+/-10years old); and subjects with diabetes mellitus (Group B, n=16, 67.2+/-8.3years old) Results: The patients from group B were admitted with significant higher serum glucose than the patients of group A (243.3+/-106.1 to 121.8+/-29.8; t=4.39, p=0.001) There were no differences in arterial gasses at admission time between the two groups. From 16 diabetic patients 13 (81.2%) and from 66 normal subjects 54 (81,8%) received oral corticoids during their hospitality. There were no difference between mean duration (in days) of therapy (GrA: 2.4+/-2.4days to GrB 3.7+/-3.8) with oral corticoids and total days of hospital therapy (GrA: 8.1+/-3.9days to GrB 8.7+/-4.8), between groups. Diabetic patients left hospital with significant lower glucose serum than that they had when they admitted (147.3+/-73g/dl to 130.8+/-52.5g/dl; p<0.01) Conclusion: Increasing blood glucose concentrations were not associated with adverse clinical outcomes in people with AECOPD, even they were diabetics or not. Appropriate therapy for diabetes during corticotherapy decreasing the concequencies of high glycose serum.
