Prognostic value of C-reactive protein in patients with severe to very severe chronic obstructive pulmonary disease

Abstract

Increasing evidence have shown systemic effects in COPD patients and its association with elevated levels of CRP as clinical marker of inflammation. Its prognostic value in relation to COPD hasn‘t clearly established while it‘s well known in cardiovascular disease.
Aim: to investigate, in severe to very severe stable COPD patients, the relationship between high-sensitivity CRP levels and factors that predict outcome.
Methods: 28 stable COPD patients (7F/21M; age:74±5) with moderate /severe airflow obstruction (FEV1: 41±7%pred) were enrolled: 19 patients with GOLD III and 9 patients with chronic respiratory failure (PaO2: 56±3mmHg) GOLD IV. Assessments included: smoking habit, number of exacerbations, BMI, spirometry, blood gases, walking test, dyspnea score, BODE index. Nobody has history of ischemic heart disease. Two groups were obtained referring to CRP values >3 mg/L which represent high risk for future cardiovascular events.
Results: BODE index was significantly increased both in patients with CRP level >3 mg/L (p=0,02) and with CRP >10 mg/L (p=0,01). A correlation was found between BODE index and CRP levels (p=0,0038; r=0,56). Serum CRP was significantly increased in patient with a BODE score of 7 to10 (p=0,0037), the highest quartile previously associated with high mortality rate. CRP was significantly increased in patients with lung hyperinflation (IC/TLC < 25%) (p=0,0036). The percentage of patients with CRP level >3 mg/L was 64%. Conclusion: CRP levels in stable COPD patients (GOLD III-IV) are well associated with BODE index. Besides established prognostic factors, CRP appears useful in the proper assessment and monitoring of patients with severe COPD.
