A rare presentation: sternum tumor which has two components

Abstract Chest wall tumors divided into two main groups which are primary and secondary. Primary chest wall tumors is 2% of all of the whole body primary tumors. These tumors are 60- 80% malign. Wide resections of chest wall are performed which reconstruction may also be necessary for surgical treatment of chest wall tumors. A 66 year old woman was referred to our clinic for chest pain and tumor in anterior of the chest in last year. A lobulated, hard lesion on the sternum, approximately 10 cm in diameter was detected in phisical examination. Thorax computerised tomographic scan demostrated a lesion at the lower segment of sternum 10 cm in diameter which involves calcifications. Fine needle aspiration biopsy and incisional tissue biopsy were performed. Result of pathology was reported as pleomorphic cell sarcoma. In positron emmition tomography, a lesion 10x11,5x15cm in diameter, involving destruction in bone has two components, one of them is at posterior of sternum has a glycolithic activity (SUV=3,5) and the other one on anterior of thorax is 8x13 cm in diameter which has solit component and increasing glycolithic activity (SUV=37,6–40,5). Malign soft tissue tumor resection, sternum resection and reconstriction with prolenmethylmetacrylate sandwich graft, wedge resection to middle lobe, bilateral fasciocutaneus fleb were performed. In the postoperative pathologic report, tumor loculated posterior of sternum was reported as good differentiated condrosarcoma and the other segment of the tumor loculated anterior of sternum extended to the skin was reported as malignant fibrous h istiocytoma. According to this the case was accepted dedifferatiated condrosarcoma. We present this case because it is seen rare in the literature.
