Effects of inpatient rehabilitation on quality of life and exercise capacity in long-term lung transplant survivors

Abstract

Background: Beneficial effects of lung transplantation (LTx) have been demonstrated to decline during the course of 2-3 years after LTx, especially with occurrence of bronchiolitis obliterans syndrome (BOS). The purpose of this prospective study was to examine the effect of an inpatient rehabilitation program in long-term (>1 year after LTx) lung transplant survivors on quality of life (QoL) and exercise capacity.
Methods: 11 LTx recipients 5.0±2.9 years after lung transplantation (age 56±3 years; 8 male; 5 double LTx; 3 BOS stage≥1) attended a rehabilitation program for 23±2 days. Patients were evaluated by cardiopulmonary exercise testing, the 6-minute-walk-test (6MWT) and the standardized ²Short-Form-36 health survey² (SF-36) self-rating questionnaire before and after (23±5 days) the program.
Results: Significant improvements were shown in sub-maximal exercise capacity measured by 6MWT (62±18m, p=0.005). There was a strong trend towards increased maximal exercise capacity without the change reaching statistical significance. Patients‘ QoL post-rehabilitation increased in all domains indicating improved self-perceived mental and functional status. Significant correlations were found between the SF-36 subscale physical functioning and peak exercise capacity (r=0.69, p<0.01) and 6MWT (r=0.72, p<0.01), respectively.
Conclusion: The study results suggest that long-term LTx survivors achieve improvements in QoL and exercise capacity by a comprehensive inpatient rehabilitation program. Follow-up data are necessary to further evaluate the sustainability of these findings.
