Abstract

The main problem of DT diagnostics is a change in radiological picture and prevalence of disseminated processes with acute clinical manifestation. The purpose of the research was studying of clinical and diagnostic importance of molecular and genetic methods as PCR-RT in 92 patients with clinical and radiological signs of DT and negativ results of respiratory material bacterioscopy for asid-fast mycobacteria. Clinical material: sputum, bronchoalveolar lavage liquid, venous blood. DNA was isolated with the help of commercial set for isolation nucleic acids ²DNA-technologia² Russia. PCR-RT was made with fluorescent detection of DNA of mycobacteria tuberculosis complex (MTC) using amplifier iCycler iQ (Bio-Rad). As a result of complex examination the DT diagnosis was made in 66 patients, including positive result of PCR-RT examination in 24 (36.4%) cases. Low sensitivity, first pf all, is caused by low quality of the samples taken for the research and was higher at the analusis of bronchial lavage liquid at fiber-optic brochoscopy, and was lower at analysis of sputum. The venous blood analysis has not revealed any positive results. Specificity of PCR-RT analysis has made 88.4%. MTC DNA was found in patients with made final diagnosis – sarcoidosis (3). Increase in analysis frequency rate and use of endoscopic means for clinical material sampling can considerably increase diagnostic informativity and clinical importance of PCR-RT analusis at DT.
