Express diagnostics of mycobacterium tuberculosis (MBT) drug resistance at disseminated tuberculosis (DT)

Abstract

DT now becames epidemically significant clinical form of tuberculosis. The purpose is to atudy an opportunity of express detection of drug resistance (DR) using fixed bacterioscopy material (113 smears from 86 DT patients). At taking of MBT cultures from these patients, the parallel research (49 cultures MBT) and comparison of the receved results were carried out. DNA was isolated from auramin-rhodamine or Z-N stained microscopy preparations (the number of acid-fast mycobacterium from 10-99 in 100 fields of microscope up to 1-10 in the field of microscope at analysis of not less than 50 fields); from MBT cultures using SDS, proteinase K, mixed CTAB2/NaCL. For express detection of DR to rifampicine (R), the mutations in codons 531, 516,526 of the gene rpoB using RNA/RNA mismatch analysis, using the set of reagents Mutations Screener (Ambion Inc.)were determined. At the analusis of MBT DNA isolated from a material fixed at bacterioscopy, basically the mutation in the genetic structure of MBT in the codon 531 (Ser531-Leu) in 40 (46.5%) persons, in codon 516 (Asp516-Val) in 9(10.5%) patients and 526(His526-Leu) in 8 (9.3%) persons was determined. 29 patients had negative results of mutations. Comparison of two research methods for revealing MBT R-resistance – bacteriolodical (with use solid mediums and method of absolute concentration) and molecular genetic detection has shown, that concurrence of the results of these methods was revealed in 82 (95.3%) patients. Plentiful MBT bacterioexcretion, registered by bacterioscopy of clinical samples is an indication for express diagnostics of DR with the help of molecular genetic methods and use of obtained results in clinical work.
