Not validated TB cases in a high endemic country – how often and why?

Abstract

Romania has the highest TB notification rate from the Eastern Europe;
In Iasi County 10462 TB cases registered 2000-2007 started anti TB treatment; 92 of these cases were finally not validated as TB.
Aim: retrospective assessment of the not validated cases, annual rate and final diagnosis.
Method: Romanian NTP allows the use of the ²invalidation² as an assessment term for the cases which aren‘t active TB.
Retrospective study of the medical records, the radiological documents from 92 ²not validated ²TB cases registered 2000-2007 to establish the causes of the invalidation.
Results:
The not validated cases represented 0, 87% of all registered cases,
57, 6% males, mean age 53, 2 years, 71, 7% new TB cases.
76% were registered as pulmonary TB, most frequent localization of cases registered as extra pulmonary TB was pleural -68, 1%.
90% from the pulmonary cases were Mt culture negative and 4, 2% culture positive.
Standard treatment regimens were used; mean treatment duration was 114 days.
The final diagnosis of the analyzed cases was: TB fibrotic sequelae 20, 6%, pneumonia 18, 4%, lung cancer 14, 1%, metastatic pleural effusions 8, 7%, other cancers 4, 3%.
Conclusions:
The number of infirmed cases doesn‘t influence the global case notification rate.
These cases are consumers from the budget allocated for TB control.
Not validated cases exist in countries with a high endemic TB rate, where pulmonologists fear the consequences of an active TB case upon an individual as well as the epidemiological ones.
In order to minimize the number of these cases, profound investigation and more time should be allocated for an accurate diagnosis and adequate therapeutically decision.
TB is ²a great mimic of all pulmonary diseases².
