Abstract

Surgical treatment of lymphangiomas is complicated by a high incidence of recurrence and local nerve damage; alternative modes of treatment are therefore under investigation. Intralesional sclerosis of lymphangioma with OK-432 (picibanil) a streptococcal derivative (Chugai Pharmaceutical Co, Tokyo, Japan), was introduced by Ogita in 1986 as first line treatment for unresectable cases. We describe a seven year old boy with lymphangioma of mediastinum initially treated with surgery. Following incomplete surgical removal OK-432 injection was used as secondary therapy. Total regression was observed. Intralesional injection of the lymphangiomas was performed under general anaesthesia. There was no recurrence after follow up 2 years. Intralesional injection OK-432 represents an alternative, safe and effective treatment for lymphangiomas. It can be used as the primary therapy, after partial surgical excision, or in recurrent lymphangiomas.
