Implementing close contact investigation in a tertiary hospital in Iran

Abstract Background: In spite of the recent experiences in tuberculosis control, case detection for tuberculosis is still unfavorable in some countries.Aim: This study is aimed to investigate tuberculosis in confirmed TB patients‘ households with whom they have had close contacts.Materials and Methods: We conducted this study for close contacts in confirmed TB patients‘ households who were diagnosed in a referral TB center in Iran 2007 -2008. Among these people, in addition to performing sputum smear studies, Chest X-ray, and Tuberculin Skin Tests (TST), a voluntary interview is conducted via a score data sheet about the personal characteristics, living environment condition, as well as the duration of exposure to the index patient (hours per week).Results: In this study a total number of 525 people were recruited. The mean age of the cases was 37±14.7 years .231(44%) were males. 32(6.1%) were illiterate VS 131(25%) with higher academic education. The majority of people were Iranians (95.6%). Of the total 525 patients, 12 (2.3%) cases were identified to have active tuberculosis. 113 (21.2%) persons had received Isoniazide Preventive therapy (IPT). In this study, latent tuberculosis infection (LTBI) did not show to be associated with age, sex, education, smoking, house area, and family size of the close contact cases. Furthermore, LTBI in close contact households did not demonstrate a significant association with smear positivity or having chest x-ray cavitary lesion in the index case. (P >0.05) Conversely, higher Contact Score revealed to be significantly associated with LTBI. (P=0.016)Conclusion: Detecting LTBI or active TB in more than 20% of evaluated individuals prompts the necessity of close contact investigation.
