Tuberculin skin tests positive children detected in 1998–1999 were re-evaluated in 2004

Abstract

Sivas Tuberculosis Dispensary‘s (Turkey) school tracings records performed to first classes of 31 inner city schools were evaluated between 1998 and 1999. Children with positive tuberculin skin tests (TSTs) were re-evaluated in 2004 with a matched control group. Targeted 2593 children, 2473 (95.4%) of them were tuberculin skin tested in 1998-1999. Among 2450 schoolchildren Bacillus Calmette-Guerin (BCG) vaccination status recorded, 14.8% (n=364) was detected as zero scar. TST positive children number was 147. The calculated annual infection risk ratio was 0.64% and, natural infection prevalence in unvaccinated children was 4.4%. These 147 TST positive children were matched with 147 TST negative children randomised from same schools in 2004. We reached 112 children in TST positive group and, 135 children in control group in 2004. In each groups, there was only one contact history in their families. There were no significant differences for household population, person per room and the health assurance status of children in each group. There was no child diagnosed as tuberculosis (TB) or latent tuberculosis infection (LTBI) in TST negative group in the last six years. In TST positive group, three children (2.7%) were diagnosed as TB and, were received anti-TB chemotherapy for six months. As it seems reasonable that remaining children (n=109) had been received prophylactic treatment, only 11 of them (10%) were started prophylactic treatment. None of these children with LTBI were completed their prophylactic treatment course. It seems that children with LTBI found by school tracings were not followed-up appropriately in our region.
