Cerebral tuberculoma in the children with tuberculosis and HIV-AIDS

Abstract A retrospective study on a group of 115 children with TB and HIV-AIDS – 12 cases presented associated cerebral tuberculomas. Results: Cerebral tuberculoma prevalence – 13.8%. Average age: 12.5 years. Sex distribution: males – 58%, females – 42%. Milieus: rural – 52.7%, urban – 47.3%. Associated clinical symptoms: fever, irritability, meningeal syndrome – 100%; convulsions – 41.4%; hemiparesis – 16.6%; clonus – 41.4%; extrapulmonary associations, hepatomegaly, peripheral adenopathies – 100%; abdominal adenopathies – 58.3%; pericarditis – 25%. Pulmonary affection types: military, meningitis – 41.4%; cavity lesions, peritonitis – 8%. Pulmonary radiological modifications: military microopacities – 83.3%; bilateral lobar infiltrate – 16.6%; paratracheal, mediastinal, hilar adenopathies – 41.4%. Decreased CD4 values <200 cells/mm3 – 41.6%; <100 cells/mm3 – 50%. Symptom onset: acute – 58.3%; insidious – 41.4%. Cerebral scanner: hydrocephalus – 16.6%; cerebral tuberculomas – 10.4%; multiple parenchymal lesions – 25%; calcifications – 3%. CRL aspect: clear – 33.3%; opalescent – 8%. Cytology: CRL, lymphocytes >90% - 41.4%. Bacteriological modifications: BAAR presence in sputum – 41.4%; CRL – 100%; sputum positive bK cultures – 25%; CRL – 8%. bK chemoresistance – 33%. HIN+RMP chemoresistance – 16.6%; HIN+RMP+SM – 16.6%. 12 months chemotherapy treatment – 33%; over 12 months – 66.6%. 12 months scanner evolution: cerebral calcifications – 41.4%; lesion resorption – 41.4%; exitus – 16.6%. Conclusions: The active tuberculosis risk in these children is 8 times greater than in the seronegative ones.
