The effectiveness of the national program tuberculosis control in Ukraine and the future strategy

Abstract

Aim of the investigation was to appreciate the effectiveness of the detection tuberculosis (TB) in high-risk groups for 1998-2004 years and the dynamic of the epidemiological indices by statistical methods. The TB incidence and mortality had increased on 46,0 % (to 80,9 per 100000) and on 44,8 % (to 22,6 per 100000) accordingly. The speed of its change had decreased – the TB incidence from 5,8 to 3,4 per 100000 and the TB mortality from 4,4 to 0,8. The TB incidence in contact groups had decreased on 25,4 % (from 6,4 per 1000, P< 0,05). The active prophylactic inspection of population by fluorography (FL) had increased on 33,4 % from 401 per 1000 and the percentage of patients from quantity of newly detected patients on 5,2 % from 48,0. The quantity of contacted people with preventive therapy had increased on 18,4 % from 76,3 %. The detection of patients with TB by smear microscopy in 2002 was low - 0,3 % from inspected people, but was higher in high risk groups (homeless at al.) from 0,7 % to11 % for 1998-2002 years. The confirmation of positive AFB by cultural method formed 38 - 35 % in Ukraine and was higher in risk groups- 80-100 %. The active detection of TB cases by FL in high-risk groups in 2002 was within the range from 5 to 53 per 1000 (higher in 41,1 times than of all population). The percentage of the people with the bacteriosecretion stopping had increased from 81,6 to 89,5 and the percentage of the people with healing of cavern in the newly detected patients with TB – from 66,8 to 77,9. Conclusion: The results of National program fulfilment was successful, but the strategy of TB control must be changed to active case finding in high-risk groups, treatment and prophylaxis.
