A comparative review of pediatric and adult cases with miliary tuberculosis

Abstract

Patients diagnosed as miliary tuberculosis (MTb) in Dicle University Hospital were retrospectively investigated. There were 26 adult and 56 child cases during a 13 years period. There was a male predominance of the patients (32/24) in children and a female predominance (18/8) in adults. The mean age of the pediatric cases was 3.9±3.3 years and the adult cases was 35.1±15.5 years. Thirty eight (46.3%) patients had family history of tuberculosis. None of the patients had prior history of tuberculosis in the past. Dyspnea, chest pain, abdominal pain, fever and night sweating were more prominent among adults; while hepatomegaly, splenomegaly, convulsion, and diarrhea were more frequent among children. In the laboratory, hypoalbuminemia, elevated levels of liver transaminases and lactate dehydrogenase were more common among adults than in children. The diagnosis was proven microbiologically in overall 15 (18.3%) patients and consistent radiological findings were present in all patients. Among 56 pediatric cases there were 8 (14.3%) children with positive BCG scar and 16 (28.6%) children with positive reaction to 5TU tuberculin test. Twenty patients (24%), 14 children and 6 adults, had tuberculous meningitis accompanying the disease. We have found predisposing factors in 13 (15.9%) patients, 11 adults and 2 children. Overall 6 (7.3%) patients had died, of whom 2 had accompanying tuberculous meningitis, and 4 with predisposing factor. To get a good suspicion of early diagnosis and treatment, different characteristics of the adult and pediatric cases with MTb should be known.
