Abstract

Aim: To asses the risk factors, particularities, the treatment and the final results of pulmonary TB relapses.
Method : We studied a lot of 91 patients with pulmonary TB relapses registered in Cluj county in 2004.
Results: The diagnosis was bacteriological confirmed in 74 cases(81,3%),most of them were males (74,7%),of active age(92,3%).From the social status point of view 48,3%had no income,4,4% were homeless, retired for other diseases-19,8%,retired-14,3%,workers-11%. Looking for associated diseases we found: 64,3% were alcoholic and smokers, COPD-32%, ulcer-14,7%, diabetes-2,19%, cancer-2,19%.
TB with extensive, bilateral pulmonary lesions was present in 35,16%. Drug susceptibility tests were initially performed to all patients, and 71,42% were susceptible to all drugs, 16,48% were MDR, resistance to one drug-8,79%, to two drugs-3,29%. The retreatment was initiated with 5 drugs in 57,47%cases, with 4 drugs in 9,89%, and individualized regimen in 9,89% cases. The final assessment of the treatment was: success rate =72% (cured=53,8% and treatment completed=17,2%) failed=13,1%, deceased=7,7%,defaulted=12,1%).
Conclusions: We consider that the standard treatment must be completed with socio-economic measures including the incentive of the pacient, the steady implication of the primary medical care, and hospitalization in MDR centers.
