Abstract

Study was conducted to assess drug resistance to various antituberculosis drugs and compare the level of M.tuberculosis acquired drug resistance in 1979-98 among various territories of Russia. Comparative computed analysis includes the annual report data which were collected from 377 Russian territorial laboratories and were based on drug sensitivity tests of more than 1 million M.tuberculosis strains. Data were collected from 84 regions with total population of more than 100 million. The isolates were tested to the drugs which are included in the standard treatment regimens in Russia: those are isoniazid (H), rifampicin (R), ethambutol (E), and streptomycin (S). The susceptibility testing of isolated mycobacteria was based on the method of absolute concentrations. The drug concentrations for susceptibility testing were: H - 1 and 5 mg/l, R - 20 and 50 mg/ml, E - 2 and 5 mg/ml, S - 5 and 50 mg/ml. Medium H resistance was reported 24.6% (from 7.8% in Kaluga region to 45.3% in Nizhny Novgorod region); R resistance - 13.7% (from 2.4% in Adygea Republic to 26.8% in Tver region); E resistance - 19.5% (from 1.4% in Tomsk region to 28.6% in Astrakhan region); S resistance - 34,3% (from 14.4% im Murmansk region to 68.6% in Tyva Republic). Multidrug resistance (H + R)was observed in 6,4-29,2% (depend on region). This results, in spite of inevitable lack of Russian laboratory service, demonstrate the problem of standard treatment regimens implementation in Russia.
The computer-based system of M.tuberculosis drug resistance monitoring is processing now as a part of State Russian tuberculosis monitoring system and the prospective study will be started in 4 Russian regions in 2001.
