Predictors of mortality in elderly subjects with and without chronic airflow obstruction

Abstract Previous studies have shown that FEV1 and other spirometric indices are good predictors of mortality in patients with chronic airflow obstruction (CAO). This study was aimed at identifying other independent correlates of mortality in elderly outpatients with and without CAO. A three-year follow-up of survival was conducted by telephone survey on the population enrolled in the SA.R.A. case-control study on respiratory health in elderly. 1241 subjects (mean age 73.8 ±] 6.5; male: 664) were included in the present analysis. 474 patients were cases, while 767 were controls. Univariate correlates of mortality were identified and introduced in a multiple logistic regression analysis. Cases had higher mortality compared to controls (20% vs 7.4%, p<0.001). The most important correlate in cases was FEV1%pred (OR: 0.97 C.I.:0.96-0.98); other correlates were lower performance on 6-min walking test (6mWT) (OR: 2.74 C.I.:1.59-4.74), age at the time of recruitment (OR: 1.07 C.I.:1.03-1.12) and male gender (OR: 2.23 C.I.:1.21-4.12). Conversely in controls independent correlates were age (OR: 1.09 C.I.:1.04-1.15), FEV1%pred (OR: 0.98 C.I.:0.96-0.99), number of drugs used (OR: 1.25 C.I.:1.05-1.49), male gender (OR: 2.87 C.I.:1.43-5.77) and depression (OR: 2.20 C.I.:1.10-4.39). The results confirm that FEV1%pred is a powerful predictor of mortality in such patients. In addition physical functioning was another important independent predictor of mortality. These results suggest that spirometry and 6MWT may provide valuable prognostic information and emphasize the importance of prevention of disability and of rehabilitation.
