Abstract Background There is a lack of knowledge on associations between long-term physical activity and respiratory health.Objective To assess the association between physical activity over time and lung function in a population-based multicentre European adult cohort.Methods FEV1 and FVC were assessed in ECRHS participants at ECRHS II (27-57 years) and ECRHS III (39-67 years). Participants were classified as physically active (> 2 times a week and > 1 hour) or non-active at both time points using self-reported questionnaires. Associations between physical activity status over time and lung function level at ECRHS III were assessed using mixed linear regression models.Results Participants who became or remained active at ECRHS III had significantly higher FEV1 and FVC levels compared to participants who were non-active at both ECHRS II and ECRHS III. Effect estimates were slightly attenuated but remained significant after excluding current smokers.Conclusions Over a ten year period, becoming or remaining physically active was associated with higher subsequent lung function in European adults. *Adjusted for age, age2, sex, height, education, smoking status and lung function at ECRHS II. Positive values indicate better lung function with respect to reference.
