Physical activity assisted by interactive video gaming as new treatment approaches in COPD rehabilitation of patients at home : Observance, satisfaction and impact on quality of life

Abstract

The aim of our study was to assess compliance, satisfaction and impact on quality of life of the physical activity assisted by interactive video game using the Wii Fit (Nintendo®), using exercise retraining protocol of the COPD patients at home.Auto-questionnaires were delivered at the beginning (T0) and the end (T1), i.e. during 12 weeks of Wii-step exercise protocol for the 3 mo-Group, or 32 weeks for the 8-mo Group, at home. 20 voluntary patients were included with a COPD (FEV/FVC<70%) GOLD level 2-4; without exacerbation since 8 weeks; with optimal medical treatment, and having a spirometry and cardiac stress tests within 3 months.14 patients were in the 3-mo Group and 6 in the 8-mo Group. We observed*78.6% of good compliance in the 3-mo Group versus 100% in 8-mo Group*64.7% of patients have estimated this device as very effective, and 35.3% as effective*94.1% of them would like to go on a long-term activityCAT quality of life questionnaire was significantly improved (p<.05) in both groups [3-mo Group: 14.6+/-6.4 at T0 versus 10.6+/-4.8 at T1] [8-mo Group: 16.5+/-5.0 at T0 versus 10.5+/-3.1 at T1]The majority of patients (85%) followed perfectly the medical recommendations and thus had a good observance. The overall satisfaction of the patients was excellent. We found an improvement in the quality of life score in short and long term. This protocol improved the feeling performances as well as quality of life, within 3 months of training. This exercise is a suitable method for chronic rehabilitation programs, in order to increase both patients' involvement and motivation at home.
