Abstract

INTRODUCTION:According to the EPI-SCAN study there is significant under-diagnosis in COPD, at around 73%. Therefore, it is essential to develop strategies to reduce this underdiagnosis.OBJECTIVE:It was to determine the percentage of COPD underdiagnosis in the area of influence of the hospital of Ourense.METHODS:Epidemiological, cross-sectional multicenter study in the health area of Ourense where a team composed by a nurse and pulmonologist attended to 16 health centers, in the period from December 12th 2012 to July 2nd 2013. Spirometric tests were performed to previously selected patients, according to the following characteristics: age> 35 years, smokers or former smokers with a pack-year index > 10, non-smokers with chronic symptoms (dyspnea, cough and expectoration of more than 6 months). We excluded patients with previous diagnosis of respiratory diseases or under any type of inhaled treatment. Main variable: % of patients diagnosed with COPD.RESULTS:A number of 405 patients from a total of 409 were selected and analyzed. From these, 35.6% (n 144) were diagnosed with COPD and 64.4% (n 261) showed no symptoms of the disease. The average age of the patients was 61.07 years (standard deviation +12, 39). 79.8% of them were males (n 115), with a history of smoking in 81% of them, and exposed to smoke in 59.7%.On the other hand, 19% (n 27) of them had no history of smoking, 41% (n 12) being women. Of the positive COPD patients, 31% show mild, and 52% moderate symptoms.CONCLUSIONS:The under-diagnosis of COPD in patients at risk is 35.6%. Most of them are diagnosed at mild or moderate stages of the disease. About 20% of diagnosed patients diagnosed are non-smokers.
