The caracteristics of multidrug-resistant pulmonary tuberculosys in women

Abstract The study includes 46 women with drug resistance, aged 20-62 years, admitted to our clinic in last10 years. During this period the total number of the hospitalized patients was 169 women. Of all treated (HIV negative) patients with secondary pulmonary tuberculos 9(19,56%) had primary drug resistance, of which 7 were chemo-resistant to one drug and 2 to two drugs. 37 cases (80,43%) had secondary resistance, of which 19(41,30%) cases were mono-resistant, 13(28,26%) cases biresistant and 5(10,86%) cases multi-drug-resistant. Among the factors involved in the occurrence of secondary chemoresistance in women, drug intolerance were predominant associated diseases and, followed by toxic substance consumption (tobacco and alcohol). Individualized therapeutic schemes were administered according to the antibiogram. Under treatment microscopic bacteriological negativity occurred in a period of 3 months in 19 patients (41,30%), the rest of the cases showing positivity 27 (58,69%). The cases had the following evolution: microscopic negativity in 11(23,91%) cases, persistence of micros-copic positivity in 13 with chronicity and multidrug-rezistant, the death is in 9 cases and the rest of the cases could not be followed up because they did not come to the periodical controls. In conclusion, drug-resistant tuberculosis in women is increasing (national statistics data 36,3%) being mainly caused by secondary drug reactions, noncompliance and multiple associated diseases. It is a major cause of chronicity and death with epidemiological implications, representing sources of contamination with resistant germs. It requires expensive medication and prolonged hospitalization with social and familial consequences.
