Abstract The purpose of the study was to follow up the evolution of chronic cases of pulmonary TB from 1997 to 2000, in the district of Cluj.In 1997 were registered 112 chronic cases of TB,in 1998 were 98 cases,in 1999 were 98 cases and in 2000 were 75 chronic cases.From the total of cases, 80% were male.The analysis of the bacteriological data shows the increasing number of drug resistant patients from an year to an other.In 1997: 41.9% of patients shows drug resistence(23.7% to H;11.6% to HR,HS,HZ; 6,1%MDR).In 1998: 39.8% of patients were drug resistents(18.8% to H,4.1% MDR).In 1999 the percentage of chemoresistence is increasing to 40.8%(17.3% to H;17.3% to HR,HS;6.1%MDR).In 2000 the percentage of chemoresistence was 46.6%(20% to H,13.3% biresistence;13.3% MDR).The success rata of treatment among this patients was very low(~25%)and the percentage of treatment failures is very high(~53%).Among the risk factors for failure is: alcoholism in 50%,noncompliance to the treatment 32%,drug resistence in 18%.The social dimension of the problems is revealed by the fact that 45% are unemployed,15% are homeless,4% are prisoners(or ex-prisoners). Conclusions.The chronic pulmonary TB is a severe form of TB,with a high frequency of MDR,hard and expensive to treat and with risk of spreading the infections.The medical measures need to be completed by social measures,psyhological support and government commitment.
