Abstract

Aim:
Establishing in concord the therapeutic conduct in drug – resistant cases in children, and especially in those with HIV/TB co-infection in the years 2000-2001 motivates the study.
Material and method:
The batch of 157 cases, with ages between 0 –16, with lung tuberculosis: 45.7% with mycobacterium tuberculosis, 17.2% have HIV – tuberculosis co-infection and 4.4% had drug resistance to izoniazid and rifampin.
The anti – tuberculosis therapy was carried out with izoniazid (H), rifampin (R), pyrozinamid (Z) and ethanbutol (E), in 85.5% of the cases; with H,R,Z in 11.1 % and individualized treatment 4.4 %.
Results:
Out of the 157 children with lung tuberculosis treated: 88.18 % healed, 5.50 % under treatment, 1.22 % failures and 5.10 % died.
In the HIV – tuberculosis co-infection group, of 26 children, with ages between 12 and 13, 15(69.2 %) had mycobacterium tuberculosis, while 7(30.8 %) with drug – resistance to H and R. Tuberrculosis treatment for the negative and positive myc/TB cases but responsive to H and R was standardised.
For H and R drrug – resistant myc/TB cases the treatment was individualized, as basedon the patient therapy history, with Z,E /S and Ethionamide Ofoxacin Amikacin.out of 7 cases, 3(1.90 %) died immediatly after the treatment was started, while 4 (2.50 %) continue the treatment.
Conclusions:
The fundamental difficulty of the individualized therapeutic management in TB drug – resistant children, leads to the idea of through – going therapeutic studies and establishing a new concord in this field.
From personal observation, it comes out that negativization of spit at direct examination and culture could be obtained by treatment applied to out of 7 cases.
