Combined nicotine patch and inhaler for smoking reduction and cessation

Abstract Aim: to assess smoking reduction or cessation in participants in an open trial with combined nicotine patch and inhaler, aimed at investigating the effect of smoking reduction or cessation on oxidant status, inflammatory response, and cytogenetic abnormalities in peripheral blood. Material and methods: 203 smokers (62% males) of 15-55 cigarettes/day, aged 45.9 (SD 9.6) years, with a Fagerström test for nicotine dependence score of 5.6 (SD 1.9) were enrolled. 190 smokers attended baseline and 7 control-visits at week 1, 2, 4, 8, 12, 26, 52. Full treatment run for 8 weeks, i.e. 15 mg/16 hours nicotine patch combined with nicotine 10 mg inhaler ad libitum up to 12 cartridges/day; patch was discontinued at week 8 and inhaler use was gradually reduced until week 26. The study is still ongoing but preliminary results are: at week 1, 94% of participants used the patch for >80% of days and inhaled 5.1 (SD 2.1) cartridges/day on average; at week 8, corresponding figures were 79% and 4.6 (SD2.0), respectively. Point prevalence at week 26 and 52 were as follows: 46% and 36% abstainers; 26% and 21% for smokers of [lte]50% of baseline cigarettes daily consumption; 23% and 36.5% for smokers of >50% of baseline cigarettes daily consumption; 5% and 6.5% for drop-outs. Conclusions: combined nicotine patch and inhaler is well accepted by smokers willing to reduce or quit smoking; more than 50% of participants succeeded in smoking reduction or cessation.
