Abstract Smoking cessation has been a major subject of medical research. We examined the efficacy of the following different methods in smoking cessation: (A) Prolonged treatment with Bupropion HCL (B) combination of Bupropion HCL and non specific psychological group support (NSGT) (C) combination of Bupropion HCL with Cognitive-Behavioural Group Therapy (CBGT), and (D) CBGT only. 327 smokers (mean age 44±]10, 168 males, 159 females) heavily depended (Fangestrom 7,3 ±] 1,9, 35 cigarettes/day) visited the clinic for counceling. 185 were randomised to follow one of the treatment groups for 19 weeks. Smoking attitude and withdrawal symptoms were recorded regularly during the treatment period. Abstinence and success rate (abstinence or 70% reduction of cigarettes) were the final end points of treatment. With the completion of treatment the continuous abstinence rates were 50 % for group A, 60 % for group B, 44% for group C and 24% for group D. According to logistic regression analysis the abstinence and success rates were lower for the group D (odds ratio 0,31, p=0,03) with no statistical difference in between the other groups (odds ratio B:1,5,C:0,8) compared to group A. Higher abstinence was observed in females compared to males (odds ratio 0,39,p=0,03). At 3 months follow up, preliminary results have shown that there was also a less success propability in CBGT group. These results indicate that prolonged treatment with bupropion HCL may offer higher continuous abstinence and success rate compared with the 7 weeks maintainance treatment, irrespectively the combination of any supportive group therapy.
