Peculiarities of tuberculosis in HIV-infected subjects

Abstract

Aim: study of TB clinical manifestations in HIV-infected persons.
We studied 60 patients with HIV-associated TB. The diagnosis of HIV-infection was made based on immunoblotting and enzume immunoassay. TB diagnosis was based on clinical, X-ray, microbiological and histologic data. Among the studied patients there were 51 males (85%) and 9 females (15%) aged 17-73. 36 patients acquired HIV-infection through intravenous drug usage, 22 ones – through sexual intercourse (12 had homosexual contacts, 10 – heterosexual). We failed to determine the infection route in 2 patients. Within 1 to 10 months, simultaneously HIV-infection and TB was detected in 29 patients, HIV-infection was detected before TB in 21 ones, and 10 patients had TB before acquiring HIV-infection. Most persons who simultaneously acquired both infections had infiltrative TB (18 ones), those who acquired HIV-infection before TB had generalized TB (11 patients) and patients with TB detected prior to HIV-infection had fibrocavitary TB (5 patients). Of TB complications we commonly observed exudative pleurisy (12 patients) and 2 patients with generalized disease developed meningitis. Of accompanying diseases, 11 patients had candidiasis and 6 – chronic viral hepatitis. 5 patients died – all males aged 25-73. Of them 3 died of TB detected simultaneously with HIV-infection and 2 of other opportunistic diseases (cytomegalovirus infection, toxoplasmosis).
