Tuberculosis in HIV-infected personas in prisons of Belarus

Abstract

In period 1998-2001 44 HIV-infected persons with pulmonary tuberculosis (TB) were treated in the tuberculosis hospital for prisoners. Only 2 observed patients had clinical signs of beginning AIDS, the rest had an obliterated HIV-infection. All the patients (males of 28±]4,2 years) were injection drug users and had been infected HIV in freedom. Tuberculosis was revealed only in 7 (16,0%) patients before prison, the rest were diagnosed as TB infected after 2-3 years passed in prison (34,7%). TB process was revealed in 68,2% patients during prophylactic examinations and in 31,8% - with appearance of disease clinical symptom. Infiltrative (61,6%) and focal (34,0%) TB forms were predominated in the detection. Destruction of lung tissue was revealed in 45,5%, bacilli shedding in 50,0% cases; primary drug resistance was detected in 31,8% and in 71,4% of them it was multidrug resistent. The leukocyte level was 7,0±]1,2·109 g/l and lymphocyte level was 28±]2,4%. 7 (16,0%) patients had progressive immunodeficiency with decrease of leukocyte and lymphocyte numbers. The patients received treatment according to DOTS and DOTS-Plus regimes with 4-5 drugs in the first stage. 66,0% patients had a positive dynamics of treatment. TB had torpid form in 12 (27,2%) patients and had progressive forms with inclination to generalization in 3 persons including the presence of AIDS symptoms. HIV associate TB in penitentiary entities demands to carry out measures complex for its timely detection, prophylaxis and adequate therapy.
