Abstract

The community-acquired (c.-a.) pneumonia incident rate was 4.8 per 1000 community population of Belarus and 14.4 per 1000 sentenced to prison in our state in 2000. Persons in prison fall ill during first or second year of serving their sentence (64.9%). They are mostly young men (under 40 years-79.3%). Basic risk factors of c.-a. pneumonia in prison are respiratory viruses (47.5%), tabaccosmoking (94.2%), weight insufficiency (61.2%), other disease (63.2%). We examined 242 patients in our prison hospital. We used a clinical index of heaviness to estimate c.-a. pneumonia (G Marchuk). Minimal heaviness was in 52.5%, emerge and maximal heaviness -in 46.5%. In estimation by the M.Fine's scale 1-2 risk degree was 81.9%. There was a lingering illnes for more than 4 weeks in 20.2%. We discovered mostly low-lobe localization (68.2%) and volume of infiltration in one segment (62.4%) on chest X-rayings. Complications (destruction, liquid in pleural cavity) were found in 22.3%, post disease changes (pneumofibrosis, dry cavity, pleural connections)- in 30.2%. Pneumonia had CD4-cell insufficiency and bad levels of adaptation (stress and similar reactions) in 90.3% (L. Garkavy). The common agents in sentenced to prison are Str. pneumonia (85.7%) with Moraxella catarrhalis (74.5%), E. coli (20.4%). The common cause of c.-a. pneumonia were found in 40.5% from sputum.
