Elevated exhaled and circulatory markers of oxidative stress in patients with systemic sclerosis

Abstract Systemic sclerosis (SSc) is accompanied by influx of activated phagocytes into distal airways. These cells release oxidants including H2 O2 that may be exhaled or induce damage to surrounding tissues with formation of thiobarbituric acid reactive substances (TBARs). We tested whether patients with SSc exhale more H2 O2 and this correlates with markers of systemic oxidative stress and clinical parameters. H2 O2 was measured in expired breath condensate of 27 SSc patients and 27 matched healthy controls. Baseline and fMLP-induced whole blood chemiluminescence, serum TBARs concentration were determined as measures of systemic oxidative stress. Exhaled H2 O2 , serum TBARs levels, baseline and fMLP –induced whole blood chemiluminescence (absolute light emission) were 3.5-, 20.5-, 1.4- and 2.3- times higher (p<0.001) in patients with SSc than in healthy controls. Current treatment with cyclophosphamide and prednisone did not decrease exhaled and circulatory markers of oxidative stress but was inversely correlated with H2 O2 exhalation (r= -0.62, p<0.001). H2 O2 levels correlated (p<0.05) with disease duration (r=0.38), time from the first Raynauds episode (r=0.44) in the whole SSc group while in patients treated with cyclophosphamide and prednisone (n=10) the association with lung involvement (r=0.72), forced vital capacity (r= -0.76), baseline chemiluminescence (r=0.72) and absolute light emission (r=0.79) was found (p<0.05). We conclude SSc patients express increased H2 O2 exhalation that correlates with selected clinical parameters and elevated markers of systemic oxidative stress.
