Abstract
With the development of fluorescence bronchoscopy photodynamic therapy (PDT) is a treatment modality of special interest. It is based on a tumour-selective drug known to lead to cell death after being activated by laser light.
In our clinic we treated 36 patients by PDT between 12/96 and 01/02, 34 men and 2 women, 63 years old (43-81 years). Indication for PDT was bronchial carcinoma (19), metachronous cancer or metastases (22). Squamous cell carcinoma was the predominant histology (38 vs. 3 adenocarcinomas/not differenciated). The prevailing early lung cancers (carcinoma in situ: 9, microinvasive cancer: 22) were treated in curative intention, the remaining 10 patients underwent PDT for palliative reasons. Follow-up data from 33 patients were available. The recurrence-free period was 17.4 months. One or more cleaning-up bronchoscopies were necessary in every patient. One patient had a scar-formation causing stenosis and making dilatation and stenting necessary. The most important side effect is an up to 6 weeks lasting skin photosensitation. But there was no relevant sunburn in our study.
PDT is a tumour selective bronchoscopical treatment being effective in the most cases of centrally located early lung cancer. The complications are low, but the price is very high.
