Results and complications of surgical lung biopsy

Abstract

Objective:
Surgical lung biopsy is considered as the gold standard for the diagnosis of diffuse parenchymal lung disease. It is more often requested since the pathologic classification of idiopathic interstitial pneumonias was newly introduced. Therefore we reviewed our results and complications of surgical lung biopsies.
Patient and methods:
From January 1990 until February 2002 68 lung biopsies (21 open and 47 video assissted thoracoscopic (VATS)) were performed ( 24 females, 44 males, aged 9 - 77 years mean 53,1 years).
The specimens were taken in 25 cases from the left side and in 43 cases from the right side (33 upper lobes (20 of them Lingula), 14 middle lobes and in 47 cases from the lower lobes.
In 28 Patients biopsies were taken from two different lobes.
Results: The overall complication rate was 10,1%. There were four deaths (mortality 5,8 %) within 30 days postoperatively. In two of them lung biopsies were taken for ARDS of unknown cause. Two other patients (2,9%) had persistend air leakage and one had massive pulmonary embolism but all three survived.
The pathological results (n)were: UIP (21); BOOP (10), NSIP (4), lymphocytic alveolitis(4), histiocytosis X (3), pulmonary hypertension (3, sarcoidosis (3) cancer (2) and others (18). In twelve cases (17 %) the definitive pathological diagnosis was not conclusive.
Conclusion:
Although surgical lung biopsy can lead to the diagnosis in most cases it may be associated with substantial risks.
